Other than the Quarterback attacking the O-Line after getting creamed by opposing DL/LBs....ummm, never.

Attack mode is a relative term. We WERE in attack mode. The problem is that our attack consisted of our OL going backwards and our QB laying on his back. Now if you mean an effective attack... well, I haven't seen one of those...
